Position Summary
We are seeking a dedicated PRN Speech-Language Pathologist (SLP) to provide one-on-one therapy to clients in their homes and communities. The SLP will assess, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders, collaborating closely with our interdisciplinary team to promote independence and quality of life.

Key Responsibilities
- Conduct evaluations to assess speech, language, cognition, voice, and swallowing functions.
- Develop and implement individualized treatment plans to address client goals.
- Provide rehabilitation for cognitive-communication and swallowing disorders resulting from neurological or traumatic injuries.
- Educate clients and caregivers on strategies to improve communication and swallowing safety.
- Collaborate with OTs, PTs, case managers, and caregivers for holistic, coordinated care.
- Document all assessments, progress notes, and treatment plans accurately and promptly.
- Utilize functional, real-world activities to promote community reintegration and independence.
Qualifications
- Licensed Speech-Language Pathologist (SLP) in the state of Georgia (required)
- Certificate of Clinical Competence (CCC-SLP) through ASHA (required)

We are seeking a PRN Certified Occupational Therapy Assistant (COTA) to join our growing team in the Metro Atlanta area. The OTA will provide 1:1 functional therapy sessions in clients’ homes and community settings under the supervision of a licensed occupational therapist.

Position Summary:
The Life Skills Coach (LSC) is a community-based position working directly with veterans and individuals who have sustained a brain injury or spinal cord injury. The LSC provides functional, real-world support to help clients increase independence, build confidence, and improve their participation in meaningful daily and community activities.
This role is ideal for someone who is compassionate, resourceful, and enjoys empowering others to reach their goals.
Essential Responsibilities:
- Work 1:1 with veterans to develop and improve daily living, social, and community participation skills.
- Support clients in accessing community resources, managing appointments, budgeting, and transportation.
- Provide hands-on coaching to help clients gain confidence and independence in their routines.
- Serve as an advocate and support system to help clients achieve individualized goals.
- Collaborate with therapists, case managers, and family members as part of an interdisciplinary team.
- Maintain timely and accurate documentation of services provided.
Qualifications:
- Valid driver’s license and reliable transportation (required)
- CPR and Basic Life Support (BLS/ALS) certification (required)
- Bachelor’s degree in a related field (rehabilitation, psychology, human services, social work, or similar) preferred but not required
- Experience working with individuals with disabilities, brain injuries, or veterans strongly preferred
- Excellent communication, observation, and problem-solving skills
- Ability to work independently, manage time effectively, and adapt to clients’ needs

The CHT will deliver specialized rehabilitation for clients recovering from upper extremity injuries, post-surgical conditions, and complex neurological diagnoses, focusing on restoring movement, strength, and functional performance.
This role offers flexibility, autonomy, and meaningful clinical engagement with motivated clients across the Metro Atlanta area.
Key Responsibilities
- Conduct comprehensive upper extremity evaluations and develop customized treatment plans.
- Provide evidence-based hand therapy interventions including ROM, splinting, scar management, and functional retraining.
- Fit custom orthoses as clinically indicated.
- Educate clients and caregivers on home exercise programs, joint protection, and ergonomic strategies.
- Collaborate with physicians, case managers, and interdisciplinary teams to ensure continuity of care.
